About half of all adults in the United States have one chronic health condition and many adults have more than one chronic condition to manage. And, chronic health conditions are among the most costly and deadly of all health conditions.

Chronic diseases are long-term health conditions that generally do not go away. Such diseases include heart disease, diabetes, obesity, and arthritis. Without proper care, they can become worse and cause loss of independence and increase disability.

Several Chronic Disease Prevention + Self-Management Programs are supported by the Nebraska Department of Health and Human Services Chronic Disease Prevention and Control Program. These programs are designed to prevent and/or control the effects of chronic illness by putting the patient at the center of the disease solution.
